Practice resource

Your PMHNP practice
billing change worksheet.

Use this worksheet with your team to identify the current owner, missing information, and next action for each billing handoff. Print it to add notes, or bring a summary to your call with George.

The examples illustrate administrative workflows, not payer requirements or Bomi service commitments. Keep patient information in your own secure systems.

01

Which clinicians, entities, or locations are changing?

Describe the current and proposed arrangement without sharing sensitive identifiers. Identify the clinician, entity, or location involved in each change.

Example: An established owner adds another clinician → list the clinician and location separately from the existing practice arrangement.

Record: Change / current arrangement / proposed arrangement / intended timing
02

What is confirmed with each payer?

Record enrollment status and unresolved questions for each payer and relevant arrangement. Keep confirmation sources and dates; do not assume participation transfers.

Example: A new location is planned → assign someone to confirm the payer’s requirements for that arrangement.

Record: Payer / arrangement / status / source and date / next question
03

Which state practice questions need separate verification?

List questions about practice authority or other state requirements for the appropriate regulator or professional adviser. Keep these separate from payer enrollment status.

Example: A practice-model question goes to the relevant state authority; a participation question goes to the payer.

Record: Question / state or regulator / adviser or source / next action
04

Which EHR and clearinghouse are you using?

Record system names, the account or access owner, and how claims are submitted today. Identify any access that would need authorization before another billing team could work.

Example: EHR → connected clearinghouse → current biller; practice administrator controls access requests.

Record: EHR / clearinghouse / submission route / access owner
05

Who owns claims and follow-up during the transition?

Separate new claims, older unpaid claims, payer requests, and patient balance questions. Name the owner for each during the proposed change.

Example: New submissions move only after responsibilities are agreed; older claims remain with an explicitly named owner.

Record: Work category / current owner / proposed owner / next review
